Wear a compact camera in a pocket or holster. Measure the amount of time from wanting to take a photograph to the shutter click. For digital cameras, this includes the time to power on, auto focus and auto light level adjustment.
A film camera might actually win.
Take ergonomic lessons from handguns for which draw speed was important. I think we need a physical "safety" to avoid taking pictures of the inside of your pocket.
