Replace Skype with an open system.  Encrypted video communication between hosts is a technically challenging but a straightforward solvable problem.

Seemingly more difficult is the distributed peer-to-peer directory service: which host refers to which person?  But that is easy, too, with already existing technology: DNS.  Every user needs a unique host name.  Subdomains might be useful.  Dynamic DNS.  NAT traversal.
