As Bruce Ivins demonstrated, those broadly in the security industry have tremendous incentive to create the threat for which they offer a defense.
This is worse than terrorism, because for terrorism, you need to recruit idle hands who have nothing better to do but believe in your cause. There is a lot of evil in the world that normally doesn't happen, because the people with skills that can do that evil can usually earn better pay using them for good. However, once there is financial incentive to create a threat, it can attract the brightest people limited only by their morals.
How much of this is going on?
Can it be prevented in general, that is, by decreasing incentives? This is a negative-sum game.
