Further investigation reveals they are all Jewish, immediately suggesting it was some sort of biological weapon, an act of TERRORISM TERRORISM fnord fnord fnord TERRORISM. Only Jewish men were targeted, suggesting the attacker was surreptitiously using a backscattering or millimeter-wave body scanner to check for circumcision.
Only the slower Jewish runners seem to be affected, suggesting the muscle composition or improved cardiovascular system of good marathoners confers some sort of immunity. This suggests the attacker has a eugenics agenda.
The actual mechanism is revealed to be much more mundane, with one original infected runner inadvertently transmitting the infection to those behind him.
