A mass of people, no one intent on causing harm, end up together causing harm. This must be a well recognized, well studied problem. What is known about it?
Does it have a name? It's not quite Prisoner's Dilemma, because there isn't too strong a component of people behaving selfishly. In fact, it might involve people attempting to behave unselfishly, to do good.
What are the exceptions? How can you tell if you are contributing toward the harm kind of situation, or in an exception?

The prisoner's dilemma can arise even when people are not selfish; their altruistic goals can conflict in the same manner as long as they don't coincide.
