The problem with 'ancient war' is that its not documented. But I like the idea of a one man on one war. Although "war is a prolonged state of violent, large-scale conflict involving two or more groups of people."
To address your post, the highest casualties documented warfare is WWII, with 72 million (with a much larger number from after the war was over). The increased number is mostly due to the destructive technology at hand at the time.
It seems to me like a boring answer to a very interesting question.
